Organizations that are involved in the business of preparing, storing, compounding and dispensing drugs in accordance with prescriptions prepared by licensed physicians. Pharmacies also counsel patients on proper use of their medication and verify that new treatments are compatible with other medicines they may be taking.
Day care centers that provide health and related services as well as care and supervision for adults who require physical rehabilitation, dementia management or other condition-specific forms of assistance on a limited but regular basis and who would be at risk of entering a skilled nursing or intermediate care facility without the support of this type of center. Ancillary services usually include meals and limited social activities.
The St. Camillus Home Care Agency is a Certified Home Health Agency that specializes in a full range of care such as skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, nutritional consultation and more.
They are certified to provide home healthcare services for physician-referred clients within Onondaga County. They work closely with patients, caregivers and physicians to help achieve goals such as enhancing health and independence. We also work with The Centers at St. Camillus, its licensed home care agency affiliate Integrity Home Care Services, and other providers to transition from one level of care to home.
St. Camillus Home Care Agency- The St. Camillus Home Care Agency is certified by the New York State Department of Health to serve physician-referred clients residing in Onondaga County. Working within the familiar surroundings of an individual’s home, they help re-establish as much independence as possible, freeing both individuals and their family to regain control of their lives. Therapies, testing and procedures can be provided in the comfort of an individual’s own home. And with people who fit right into nearly any lifestyle. Most of their clients are on a first-name basis with their team members. They consider them first as “part of the family,” and second as the highly-skilled therapists, nurses and aides they are. Programs that offer skilled nursing care under the guidance and supervision of a physician in the homes of recently discharged hospital patients and other people who need continuous nursing care but are not in an acute phase of their illness.
Provides assistance with enrolling in health insurance on the NY State Health Exchange, as well as help in resolving problems with your health insurance or health care, or finding health care and accessing financial resources to assist with obtaining health care services or prescriptions.
EISEP is a New York State Office for the Aging program, administered by CCFCS, designed to assist older persons who need minimal assistance with housekeeping and personal care. The EISEP Community Collaboration has case managers at four different sites - Catholic Charities Family and Community Services (CCFCS), Urban League of Rochester, Lifespan, and Jewish Family Service.
Services include:
Case management (in-home assessment, linkage to appropriate services, monitoring of services), in home personal care (bathing/dressing), housekeeping (light cleaning/laundry/meal preparation). Non-medical home support services.Non-institutional respite care (EISEP contracts with four social adult day programs to provide services to eligible participants)
Social workers assess each family's needs and assist in obtaining assistance from government programs, national and community agencies, advocate for families regarding medical insurance issues, provide financial assistance for illness-related expenses, and act as a referral source for counseling. Not applicable
Programs that offer the services of paraprofessional aides who provide personal health care services which do not require special technical training, in the homes of recently discharged hospital patients, elderly individuals and people with disabilities. Services are provided in accordance with a written home health care plan and may include feeding, bathing and grooming patients; changing their beds; taking their temperature, pulse or respiration; helping them to the toilet or to use a bedpan; and other types of assistance that enhance their physical and emotional comfort. The home health aide may also perform other activities as taught by a health professional for a specific patient including changing a colostomy bag; assisting with the use of devices for aid to daily living; assisting with prescribed range of motion exercises; assisting with prescribed ice cap or collar; doing simple urine tests for sugar, acetone or albumin; measuring and preparing special diets; measuring fluid intake and output; and supervising the self-administration of medications (reminding the individual to take the medication, opening bottle caps, reading the medication label to the individual, observing the individual taking medications, checking the self-administered dosage against the label of the container and reassuring the individual that they have obtained and are taking the correct dosage).
Referrals are received from Child Protective Services on cases needing further support and services throughout the county. The program focuses on a family's strengths, rather that the CPS report. The goals of the program are to prevent the need for future CPS involvement, help families develop; neighborhood and community-based supports; and to engage the families who are experiencing challenges early and provide support to build family resiliency.
