Care Management services are person-centered services designed to assist individuals who have barriers to access resources, supports and services due to symptoms from a mental health condition. These services can assist one to navigate and connect to providers, resources and tools in the community that may help in maintaining recovery and self-sufficiency.
Care Managers are trained, experienced professionals who can help one develop a plan of intervention, meet with one regularly to assist in addressing goals and they can advocate for one with various providers throughout Ulster County. Some challenges/barriers that Care Managers can help with are:
