    An unrelated care provider who resides in the same household as the HCBS waiver participant and provides as needed supports to address the participant's physical, social, or emotional needs in order for the participant to live safely and successfully in his or her own home.

    Eligibility:People seeking OPWDD services must contact the Regional Front Door in your area first to obtain eligibility. Once eligibility is determined, the Front Door will help you figure out which services are best for you and get you into services as quickly as possible. If you have a service coordinator, they will help you too. People who already have OPWDD eligibility can work with their service coordinator to change a service or get a new one. If you do not have a service coordinator, the Front Door will help you get one.
