Our specialty is helping people remain in the community - avoiding unnecessary trips to the emergency department and hospital, and delaying or avoiding nursing home placement.
With recent advances in technology, our team of doctors, physician assistants and nurse practitioners provide the same primary medical care in your home that you would receive in a traditional doctor's office.
In addition to the personal, quality care delivered in your home, you also have access to our providers by phone - 24 hours a day, 7 days a week, 365 days a year.
Our services can be used for just a few months to help get you back on track if you've been in the emergency department/hospital frequently. Or, if you regularly require in-home services or have a physical condition which makes it difficult for you to leave your home, you may qualify to receive our services on an ongoing basis.
We specialize in caring for people with chronic conditions such as:
*Respiratory, including COPD and pneumonia
*Cardiac, including congestive heart failure
*Diabetes
Insurance
We are a Medicare and Medicaid provider and accept most private health insurance plans. The cost of this service is the same as an office visit. We simply bill your insurance as a primary care provider. You may pay a co-pay if your insurance requires you to do so.
