Family Health Plus

  • Description:

    FHP is a Medicaid program for adults who are above Medicaid income and resources but do not have access to health insurance. Applications can be made at the Department or through Facilitated Enrollers (North Country Prenatal/Perinatal Council at 1 (800) 279-8679). If you are found eligible, Medicaid pays monthly health insurance premiums to either GHI or Excellus Insurance companies who provide your actual health insurance coverage. The comprehensive health coverage includes: Inpatient and Outpatient health care, Physician services,Radiation therapy, chemotherapy, hemodialysis, diabetic supplies and equipment, drug,alcohol mental health (some limits apply) emergency ambulance services, durable medical equipment, prescription drugs and smoking cessation products lab tests, X-rays, vision, speech and hearing services, rehabilitation services (some limitations apply) hospice and dental (GHI only).
