Description:Bridges to Health (B2H) is a Medicaid Waiver Program designed to supplement NYS Medicaid state plan services to populations of foster care youth, their families and caregivers. Foster care children and youth may have medical/physical, emotional and/or developmental challenges. The program works to support children and caregivers in the lowest level of care possible including return to home and/or adoption. The goal of the program is to support youth as they reach their highest level of potential, attain permanency and develop social and emotional competencies that provide for healthy, permanent families.
Available Services
Parsons Case Management/Health Care Integration, Skill Building, Family Caregiver Supports, Day Habilitation, Pre-vocational Services, Supported Employment, Planned Respite, Community Advocacy and Support, Crisis Avoidance, Management and Training, Crisis Response and In-home Support, Accessibility Modification; Adaptive and Assistive Equipment. Families benefit through direct services provided by trained staff, as well as staff of other local agencies who have entered into contracts with Parsons to participate as waiver service providers.

Eligiblity:•Youth/families in foster care placements of 12 beds or less or on trial discharge.
•Youth who are Medicaid eligible
•Youth with qualifying diagnoses serious Emotional Disturbance, 19 years of age or younger, , Developmental Disability 21 years of age or younger or with Medical Fragility 21 years of age or younger and who originate from Albany, Clinton, Columbia, Delaware, Essex, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, St Regis, Saratoga, Schenectady, Schoharie, Warren or Washington counties. Services begin when a child/youth is in an eligible foster care setting and may continue post discharge from foster care.
