Program Bridges to Health (B2H):

Bridges to Health (B2H)

  • Description:

Bridges to Health (B2H) is a Medicaid Waiver Program designed to supplement NYS Medicaid state plan services to populations of foster care youth, their families and caregivers. Foster care children and youth may have medical/physical, emotional and/or developmental challenges. The program works to support children and caregivers in the lowest level of care possible including return to home and/or adoption. The goal of the program is to support youth as they reach their highest level of potential, attain permanency and develop social and emotional competencies that provide for healthy, permanent families.
