States offer a range of Medicaid home care service programs, each oriented toward slightly different needs and sometimes to different populations. Often, benefits may only be accessed through Medicaid Managed Care Plans and Medicaid Long Term Care Plans. All Metro works closely with almost all Managed Care Plans in our markets.
Medicaid Service Coordination (MSC) is a program that provides ongoing support to individuals with developmental disabilities and their families.
New York State program for seniors administered by the Department of Health. Income-eligible seniors aged 65 and older may receive assistance to supplement their out-of-pocket Medicare Part D drug plan costs. Seniors can apply for EPIC at any time of the year and must be enrolled or eligible to be enrolled in a Medicare Part D drug plan to receive EPIC benefits and maintain coverage.
EPIC provides secondary coverage for Medicare Part D and EPIC-covered drugs purchased after any Medicare Part D deductible is met. EPIC
The Open Doors Project is a nursing home, hospital, and psychiatric center de-institutionalization program. Subcontracting through the New York Association on Independent Living (NYAIL), and providing services in St. Lawrence & Jefferson County, NY. This project is designed to provide consumers, social workers, discharge planners, and family members with objective information about possible home and community based services that may be available. In addition, the Transition Specialist works with the discharge planner to obtain support services to enable an individual to return to living in the community. Examples of available services may include:
• Senior Care Services
• Home Health Care options
• Housing resources
• Medicaid Waiver Programs
• Financial Assistance Programs
• Mental Health Services
If a resident or their family is considering transitioning the resident out of an institution, the Transition Specialist can meet with the resident and family to discuss their needs. If these services and supports needed are available, the resident and/or their family can decide whether or not to start the plan to leave the facility. The resident or family can change their mind at any time.
The Transition Specialist can also attend discharge planning meetings, advocate for community supports, and provide post-discharge assistance.

The Department of Social Services (DSS) provides Medicaid for Nursing Home/Chronic Care, which is determined through eligibility for Nursing Home and Spousal Long Term Home Health Care with waivered services for spousal impoverishment cases

The Health Insurance Information, Counseling, and Assistance Program (HIICAP) is available through trained counselors located in local offices for the aging across the state. Counselors are available to answer New Yorkers’ questions about Medicare, Medicare Advantage programs (managed care), Medicare prescription drug coverage, Medigap, and other health and long term care insurance.
EPIC Program participants can save a substantial portion of their costs for prescription drugs. Recent changes have simplified the Program. Use EPIC for even more savings with the Medicare Prescription Drug Program.
