NY Connects - Steuben County

  • Description:

    NY CONNECTS serves as the aging and disability resource center for Steuben County community partners.

    It is a single point of entry for people of all ages in need of long term care services regardless of income levels. We screen for access to public benefits, are familiar with most eligibility requirements, and offer application assistance. We are a trusted place where people of all ages can get reliable information on all long term care services, including help putting together a plan to remain as independent as long as possible in the community. This is referred to as Options Counseling.
