Medicaid is an income and resource eligible program for New Yorkers who can't afford to pay for medical care. Services covered include inpatient and out patient hospital care, physician services, skilled nursing care, laboratory and x-ray services, optical, dental, early and periodic screening of children, home health care services and medical transportation services. You must use providers who accept Medicaid.
It is not necessary to appear in person for a Medicaid Interview. Persons may call 315-798-5511 to have an application mailed to them. Applications may be retuned by mail with required documentation and eligibility determination will be made and the individual will be notified of their eligibility. Applications may also be obtained in person at 800 Park Avenue, Utica NY 1st Floor or 300 W Dominick St, Rome. People under 65 years of age and not in receipt of Medicare can apply through New York State of Health at 1-855-355-5777 or www.NYStateofhealth.ny.gov
