    Eligiblity:Patients cannot be insured- this includes private insurance, Medicaid, Medicare, Family Health Plus, or have a Medicaid spend-down. Only patients of the clinic will be provided with sample medication. If samples are not available, the patient will receive a prescription that can be filled at a discount rate from any drug store in Massena. Medication can be applied for from some pharmaceutical companies, but a copy of most recent W-2's is absolutely necessary. Medication prescribed by anyone other than the clinic physician will not be seen by the clinic. If you arr applying to become a [patient of this clinic and need medication you must provide the clinic with your records from your previous attending physician. Patients must be 19 years of age or older.
