Eligibility Requirements

  • Documents:The health care providers at Bassett believe that parents provide a most important background for their child's health and wellness, enabling us to give the best possible care to children. It is therefore our policy that a parent or parent's proxy be present whenever we treat minors, except as permitted under NYS law. If unusual circumstances prevent you being present when your child is seen, then we will need a signed copy of either: Consent by Proxy for Non-urgent Pediatric Care (authorizing someone else, who accompanies your child, to make health related decisions for you regarding your child's care) or Pre-authorization to Treat Minors Consent (giving us a one-time OK to see your unaccompanied child)
