  • Description:

    POTS’ Case Management team can assist you with public benefits (SNAP and health insurance applications on-site), supportive housing, and general advocacy. POTS also provides referrals to detox and outpatient treatment, shelters, employment, financial counseling, educational, and other programs.

    Eligiblity:All clients need to meet with a Case Manager for an intake application. Clients need to have a picture ID and can only do a walk-in on Monday, Tuesday, Wednesday or Friday from 9:00AM-3:00PM.
