This 24 hours a day program assists persons with serious mental illness and/or medical conditions in obtaining access to mental health treatment, medical, vocational, financial, housing and social services. A Care Coordinator communicates with clients' other care givers to assure that they receive the services they need to stay out of the emergency rooms and hospitals. Assisted Outpatient Treatment (Kendra's Law) is a separate component that utilizes the Courts to mandate treatment for individuals who are determined to be non-compliant with medications and treatment and are at imminent risk to self or others in the community.
