Montefiore Home Care provides vital services to patients in the Bronx and lower Westchester communities. Members of the Home Care team work closely with discharge planners, physicians and nurses to create a means for newly released patients to receive continued access to critical services and treatments in their home.
Our licensed healthcare professionals help patients with their daily activities and better manage their health conditions by providing access to:
Professional nursing
Physical therapy, occupational therapy and speech therapy
Wound care
Social work services
Diabetic management
Physical Therapy Program for patients – S/P Joint Replacement
CHF Specialty Program
Programs that provide equipment that is used to clean the blood of patients in whom one or both kidneys are defective or absent, and to remove excess accumulation of drugs or chemicals in the blood.
Provides home-based care to those patients suffering from acute or chronic pain. Focus is on the management of pain and other physiological, psychological, social and spiritual problems. Other specialized medical services consists of medical suergical care, advanced wound care, ostomy care and pallative care. Complementary care services are also available, including massage, acupuncture, and reflexology. There is also Handicap accessible without special facilities, Restrooms have full wheelchair access
Providing care in the home. The following professional health care services are available as prescribed by your physician:
*Case management (care coordination by our nursing staff).
*Infusion therapy.
*Wound care management.
*Diabetes counseling.
*Education to promote independent living. *Medication management.
*Licensed physical therapy, speech therapy, occupational therapy, medical social work, nutritional services and home health aide visitation provided in conjunction with skilled nursing and case management.
UR Medicine Home Care certified home health aides (HHA’s) work in conjunction with skilled nursing care. They assist with hands-on personal care needs and activities of daily living for patients with an acute illness, chronic conditions or disabilities. Services include personal care and grooming, light housekeeping, transfers in and out of bed, bathing and dressing, meal preparation, medication reminders, running errands, grocery shopping and laundry services.
Offers nursing, hospice and palliative care for those with terminal cancer and have no resources to pay for health care. Disabled parking, ramp, elevator, wide doors, accessible bathrooms
Home Instead assists older adults one-on-one with everyday tasks. Services include: meal preparation, medication reminders, laundry, light housekeeping, escort for shopping and errands, respite care, and companion aide.
This program routinely investigates and aids in the prevention of the spread of reportable communicable diseases through investigation and education by Prevention Nurses Team and Environmental Health Staff. This involves interaction with county residents, medical facilities, nursing homes, local physicians, and other county health departments. It includes the following activities, as well as STD and TB testing and treatment:
Disease investigation and surveillance
Disease outbreak control and case management
Immunizations against infectious disease
Prevention and treatment of sexually transmitted disease
Tuberculosis screening and investigation
