Social Work staff assess the needs of clients and identify services and resources that enable the individual to remain in the community. Case management includes referring and arranging services, monitoring and coordinating programs and providing supportive counseling. Home delivered meals and local transportation may be arranged. This program also provides information related to government benefits such as SCRIE, SSI, Food Stamps, Medicaid, and Medicare Part D. Additional services include referrals for light housekeeping, cleaning, shopping, bathing, dressing, and home delivered Kosher or non-Kosher meals.
