New Eyes purchases new prescription eyeglasses for low-income individuals in the U.S. through a voucher program.
Family Health Plus provides health coverage to adults who do not have insurance through their employers, but have income too high for Medicaid. Parents and childless adults aged 19 through 64 are eligible if they live in New York State, have no health insurance coverage and have income at or below the eligibility levels established by the New York State Department of Health.
The Health Home program program provides coordinated care and support for people with complex medical or mental health issues on Medicaid. Care Coordinators work with the individual on setting and meeting goals in all aspects of their life: attending medical appointments, doing recommended follow-up, housing, employment goals etc. The plans are individualized to meet the person’s unique needs.

www.nystateofhealth.ny.gov is the site New Yorkers can access to enroll in health insurance. This site is also where small businesses can research plans / options. The site is secure and easy to access. At the end of the process you will have the option to choose your plan and carrier, with easy to understand descriptions of each one. You will also see the costs for various insurances and you will find out if you are eligible for Medicaid. Nursing Home Unit, a division of Medical Services, is available to help applicants their families, and/or their representatives to apply for MA to cover Nursing Home expenses. Under MA regulations, a nursing home resident is allowed to keep $50 per month for their personal needs and enough money to pay for the cost of the residents third party health insurance; all other income must be paid towards the cost of their nursing home care. (Some of this income may be kept for support of dependent family members). If there is a spouse remaining in the community, resources and income above the established Medicaid levels may be kept. (Please refer to the Skilled Nursing Facilities Section of this Directory for further information). To apply for this type of MA call 379-2189 and ask the Nursing Home Unit Supervisor for an application.
